More conventional centres only provide information to the patient. In contrast, Medgate can in 57% of all patient contacts successfully treat the patient and prescribe medicine via a mail-order pharmacy. A team of 90 physicians and medical assistants handles 2,000 patient contacts daily and takes care of around 2.8 million insured persons via telephone, internet and biometric monitoring.

Implemented in the Andalusia region of Spain, Receta XXI is an eHealth solution that automates all processes involved in prescription, control, and dispensation of medicines. It also assists in other health services-related administrative tasks, like billing.

Objectives and purpose


Receta XXI’s objective is to improve all processes involving medical prescription and dispensation as also medical products using advanced information and communication technologies. It aims to:
- Facilitate the tasks of physicians and pharmacists by saving time and reducing administrative burdens.
- Avoid visits to the doctor’s office, especially for the chronically ill.
- Increase ‘patient safety’ by substituting error-prone manual processes with automated ones.
- Allow policy to better control medicine expenses and align it with regulations.

Detailed description


Receta XXI provides electronic support for all Andalusian pharmacies regarding medicine prescription and dispensation. Receta XXI works in tandem with the Citizen Electronic Health Record (DIRAYA) and the Patient Health Card identification system. It inter-operates with applications of pharmacies, providing a secure and safe digital environment. Receta XXII, e-prescribing system has the most cumulated practical experience in Spain and probably Europe. 
The solution supports the entire gamut of medical prescription and dispensation to patients. It also covers issues like identification (user authentication), prescription, dispensation and invoicing.
When patients consult a physician at the primary healthcare centre, they present their electronic health card which contains their identity, insurance status and rights profile. The card allows the physician access to the Citizen Electronic Health Record, which includes an electronic prescription module. The physician uses this module to generate an electronic prescription that is registered in the Central Medicine Delivery Module (MCD) of Receta XXI. In addition, the system generates a paper information sheet for patients with details about prescribed drugs, dosage, duration of treatment and other recommendations.
After the visit, patients can go to any Andalusian pharmacy with their electronic health card and the pharmacist can, check the prescription online and if everything is okay, deliver the prescribed drug. The application is inter-connected with the pharmacy management system as also with the General Pharmaceutical Council system for billing and accounting services.
If necessary, the pharmacist can contact the physician to discuss any related events.
During an ongoing treatment, patients do not require to consults the doctor solely for a new prescription. They can simply go to any pharmacy with their electronic health card to obtain the prescribed medicine(s). This is particularly convenient for chronically ill patients. For example, an old lady suffering from diabetes only needs to visit the doctor once to obtain her ’electronic prescription’ for insulin. She can then go directly to any pharmacy, even if in another Andalusian city, to obtain the medicines by presenting her health card.
Receta XXI was started in October, 2003. According to the Andalusian Health Service (7/2007), the system is in operation in 422 primary care centres, involving 4,000 physicians, with 5.5 million registered patients and 3,012 pharmacies. It has reduced the visits of chronically ill patients to healthcare centres by 22%. SAS reports that more than 25 million prescriptions have been delivered by the system till date. It is estimated that total deployment in entire Andalusia will be achieved during 2008.

Receta XXI is funded by the Andalusian Health Service.

Interoperability and standards


RECETA XXI is interoperable for information exchange within the digital platform Andalusia Health System. It works with DIRAYA and the Electronic Health Card system. It is interoperable with most information and communication systems in operation at pharmacies. Data communication is based on the HL7 standard. The system is intended to be compatible with any other prescription system in or out of the region, offering secure exchange with both, the dispensation and billing module.

The system is based on four technical prerequisites:
- Wide band corporative network
- Citizen Digital Health Record (DIRAYA) system
- Informatics applications at pharmacy offices
- Security measures for access control and secure communications
RECETA XXI is built on three functional modules:
- Prescription module: Integrated in the Electronic Health Record System (DIRAYA)
- Dispensing module: Web application integrated in the pharmacy management application.
- Checking module: Web application serving users to validate prescriptions that require prior approval.

Results
Outcomes


As of July 2007, Receta XXI was implemented at 28% of all primary healthcare centres, involving 73 % general physicians and 88% pharmacies. Almost 70% Andalusians benefited from this service. The electronic prescriptions issued represent 17% of all recorded prescriptions in 2006. Evaluation data of the North of Cordoba Health Area shows savings of about 22%.

Wider impact


Patients:
- Reducing the number of visits to physicians’ for prescription renewal in long term treatment, particularly for chronically ill patients and those living in distant places.
- Possibility to obtain medicines in any pharmacy in the region.
- Reduction in the number of errors.

Pharmaceutical Professionals: Better and easier communication with physicians.

- Reduction in pharmaceutical expenses.
- Simplification of administrative tasks.
- Real time availability of information for decision making.

Results expected?


In general, available data confirms that actual results are in accordance with expected results. However, the implementation rate is slower than initially foreseen and extra efforts are required for information and user education. In addition, different levels of usage adoption have been detected at different sites.

Success factors


- Driving political impulse and budget allowance from high level healthcare authorities.
- Agreement with pharmacists association (Consejo Andaluz de Colegios Oficiales de Farmacéuticos) from the starting phase.
- Technical competence by implementing institutions.

Failure factors


1. The Receta XXI team underestimated :
- The complex relationship between the involved stakeholders (physicians, pharmacists, the Pharmacists Professional Organization, other healthcare organizations and patients).
- The tendency to abide by a traditional operation culture.
- The effort(s) needed for user education and information.
- The need for effective change management.
- Problems related to telecommunication infrastructures, especially during the starting phase.
- During the learning phase, user(s) often went back to paper-based prescriptions.
